How to Create a Submission Answer SPI

What is it?

The ability to create SPIs based on data collected on form submissions.

Who can create/manage submission answer SPIs?

Any user who is a Form Owner can create and manage these SPIs for the forms they own.

How can these SPIs be created/managed?

Submission Answer SPIs can be created in the Safety Performance Management (SPM) feature.

Select Safety Performance under Reports

Select the Safety Performance Indicators tab.

From the New Submission SPI dropdown, select +New Submission Activity SPI.


Here you can name the SPI, set caution and warning levels, and add data entries.


Complete the following fields to create the SPI:

  • Title: Display name for SPI
  • Published?:
    • Toggle "Yes" to allow SPI to show on the Safety Performance Monitoring tab of the Quick Risk Safety Profile (QRSP).
    • Toggle "No" to not have SPI show on the QRSP.
  • SPI Type: Select how you want the SPIs to be measured.
    • Available Options are:
      • Answer Percentage
      • Answer Count
    • The following answer types pull from data entered in the Operational Logs feature. Multipliers can also be added to these options.
      • Answer Per Flight Hour 
      • Answer Per Flight Day
      • Answer Per Cycle
      • Answer Per Labor Hour
      • Answer Per Aircraft Movement 
  • Caution & Warning Levels: (optional)
    • Indicates the level of criticality for exceeding thresholds. 
    • The system monitors these safety indicators within the organization. Should the monthly value exceed the set thresholds, form owners will receive a monthly notification of these exceedances.
    • Note: Setting a warning level lower than a caution level means an exceedance happens when the monthly value is too low.
      • For example: An organization has a safety objective to have an average of 20 submissions per month. To monitor this objective, they would create a submission activity SPI, set the Caution Level to 15 and the Warning Level to 10. Form owner will receive the monthly notification alerting them that they are below their goal. 
  • Caution Level: Set a desired caution level to appear on the SPI graph. This will appear yellow on SPI gauge/graph.
  • Warning Level: Set a warning level to appear on the SPI graph. This will appear red on SPI gauge/graph.
  • Form: Select the form for which to track the submission answer data.
  • Section: Select the appropriate section on the form where the answer is located.
  • Question: Select the appropriate question on the form.
  • Answer Option: Select the answer the SPI will track.


After entering all relevant information, click Save. You can then view your SPI as a graph in the SPI tab. 


The SPI can now be tied to a Safety Objective or to keep as a monitoring SPI, leave the SPI as is.
