Submitting a Risk Assessment (RA, FRAT, etc.)

What is a Risk Assessment in Baldwin? 

Risk Assessments are used for daily operations and to help support the SMS component of Safety Risk Management. The Risk Assessment form in your portal may vary slightly from our Template but all Risk Assessments in the Baldwin system look and function alike.

How do I Submit a Risk Assessment?

On a Desktop 
  • Access the Risk Assessment by using the left hand navigation menu.
    • You can also save the Risk Assessment as a Favorite to quickly create a draft from your Dashboard.
  • Click on the “+” beside the Form name to Create a New Submission.
On a Mobile Device - 
  • Click on the Green Plus Sign at the bottom right corner.  Clicking this button will give you a pop up with a listing of the forms that can be submitted.
  •  If you need to add additional crewmembers or other individuals to an RA, they will need to be added as a Contributor. This will give them the same access as the Submitter so they can edit the form and be able to view it in Draft mode

  • At the very top of the form is the multi-colored Horizontal Risk Bar. This works off of the Risk Matrix that appears on some of the other tools in the Baldwin system.

  • The Horizontal Risk Bar works off a scale of 1-5. Level 1 being the lowest (Dark Green) and Level 5 being the highest (Red).
  • As you populate the items, the black arrow to the left of the Horizontal Risk bar at the top, will move to the left or right based on the point values associated with each item. The overall risk score at the top left corner will change, as well.

Additional Information:

• Red * denotes a Required Item. You cannot submit the form until you populate this field.

• Date Fields: Must be populated by using the Calendar feature. Typing in the field will not register data.

• Asset/Aircraft Field: Pulls from your organization’s aircraft/vehicle database. Simply start typing the name or tail number of the asset and it will appear in a dropdown menu for you to select.

• Location/Destination Fields: Pull from our airport database. Simply type a few letters of the city, state, airport name, or airport ID and a list of airports will appear in a dropdown for you to select.

• Other Considerations: You can also add Risk Reduction items in the Other Considerations area. Simply type in a description, select a Point Value, and a Risk Value from the dropdown menu.

• Organizational Comments: Have “on the record” discussions by posting your comment in the section. Anyone associated with this form will get notified of a new comment for them to view and comment back.

• Automatic notifications: Are automatically sent to a Manager for form submissions with overall Risk Scores of 3, 4, and 5. (These notification triggers may be different for your organization depending on whether they requested customizations or not.)

• Combinations: There are “behind the scenes” combinations that add additional risk points to the Risk Assessment when both items are selected. (For example, if Flight Type is Maintenance Test Flight AND a Flight Training/Proficiency flight, an additional amount of points will be added.)

• Closing My Risk Assessment: Submitters can close a Risk Assessment with a score of Level 1 or 2. A member of management will need to close forms with scores of 3 or higher.
