Request changes to your forms
What is it?
The ability to request a change to a form inside of the Baldwin portal and communicate with the Baldwin team on your requested changes. You can also view how your form is configured on this page. Please visit this article for more information on the form configuration area of the Change Requests page.
Who can request changes
Anyone in a Form Owner administrative role with assigned forms
Form Management is located under Form Management in the main menu. All of the forms where you are listed as a Form Owner will appear on this page.

How do I request a change?
- From the main Form Management page, click Change Requests to the right of the form you want to update.

- Each section and question on your form will have its own Change Request button where changes can be requested.
- Selecting a Change Requests button will open the following panel to the right of the page.


- Click + New Change Request to create a new change request.

- When your change request is created, the Baldwin team is notified and will complete the change request as soon as possible.


- Questions or sections with open change requests will also appear on the main Forms page with a red circle and the number of open requests.

- You will be notified once the change request has been completed (depending on your notification preferences) and the request will be marked complete.
Are there additional views?
Yes! You can elect to view your change requests in a List View by clicking the List View button at the top right of the page.

This will open up a list of all changes made to that form via the Change Request feature.
